Residential Criteria & Screening Process
● Application process takes up to 3-business days. We will call you as soon as we know if the application is approved or denied.
● Application fee is $40.00 for singles and $70.00 for legally married, fee is non-refundable and must be paid with cash or a money order.
● If married but do not have the same last name you will be required to pay for 2 single applications or provide a marriage certificate.
● Each person 18 years of age or older that will be living in the property will have to fill out their own application.
● All applicants will be screened for the following: credit check, criminal background, rental history and job duration. If you owe a previous landlord, utility company or have any evictions you will not be eligible to rent from us.
● Applicants can not have any misdemeanors within the last 5 years.
● Applicants can not have any felonies within the last 10 years. (Applicants will be automatically denied if the offense is-Assault, Sexual Assault, Burglary, Robbery, Evading Arrest, Criminal Trespassing or Drug Related. All other are at the discretion of the property manager.)
We will not lease to anyone under the age of 18.
● Monthly gross salary must be 3 times the rent (before taxes).
● Please provide copies of your last months total income (paystubs, disability awards letter, child support, government assistance, etc.).
Photo Identification and Social Security Card is required for all persons over the age of 18.
● Renters Insurance is Required.
● On properties that allow pets, there is a $300 non-refundable pet fee per pet (limit 2). No aggressive breeds will be allowed and no snakes. Proof of current shot records are required upon application approval.
● Deposits (security and pet) and first month's rent must be paid with money order or cashier's check. All deposits must be paid in full before move in.
● We must have account numbers for water, gas & electricity prior to signing the lease.
● Upon approval, you will be asked to bring in your security deposit. This will hold the property for up to 2 weeks. At this point your deposit is not refundable.
● For your health and safety, all of our properties are non-smoking.
1. Rental History - We will check with your current landlord and your previous landlord. Any negative feedback could result in grounds for denial.
2. Credit Check - We may overlook student loans and medical items, but take all other slow or late payment history into consideration. Any items for past landlords or any utility company could result in grounds for denial. We may make an exception if we are provided a confirmed and current payment schedule.
3. Work History - We will confirm dates and job titles for your current and past employer.
4. Current Income - We will require confirmation of current employment, along with proof of income. One month of income in the form of stubs or deposit records may be sufficient.

We will need payment of application fee before we can start the application process.
Residential Lease Application
Each occupant and co-applicant 18 years or older must submit a separate application.
Applicant is strongly encouraged to view the Property prior to submitting any application. Landlord makes no express or implied warranties as to the Property's condition. Applicant requests Landlord consider the following repairs or treatments should Applicant and Landlord enter into a lease.

Authorization: Applicant authorizes Landlord and Landlord's agent, at any time before, during or after any tenancy, to:

(1) obtain a copy of Applicant's credit report;
(2) ontain a criminal background check related to Applicant and any occupant; and
(3) verify any rental or employment history or verify any other information related to this application with persons knowledgeable of such information.

Notice of Landlord's Right to Continue to Show the Property: Unless Landlord and Applicant enter into a separate written agreement otherwise, the Property remains on the market until a lease is signed by all parties and Landlord may continue to show the Property to other prospective tenants and accept another offer.

Privacy Policy: Landlord's agent or property manager maintains a privacy policy that is available upon request.

Fees: Applicant submits a non-refundable fee of $40 to Lewis Realty for processing and reviewing this application. Applicant will not submit an application deposit if a lease is not executed.

Acknowledgement & Representation:

(1) Signing this application indicates that Applicant has had the opportunity to review Landlord's tenant selection criteria, which is available upon request. The tenant selection criteria may include factors such as criminal history, credit history, current income and rental history.
(2) Applicant understands that providing inaccurate or incomplete information is grounds for rejection of this application and forfeiture of any application fee and may be grounds to declare Applicant in breach of any lease the Applicant may sign.
(3) Applicant represents that the statements in this application are true and complete.

(1) to my current and former employers to release any information about my employment history and income history to the above-named person;

(2) to my current and former landlords to release any information about my rental history to the above-named person;

(3) to my current and former mortgage lenders on property that I own or have owned to release any information about my mortgage payment history to the above-named person;

(4) to my bank, savings and loan, or credit union to provide a verification of funds that I have on deposit to the above-named person; and

(5) to the above-named person to obtain a copy of my consumer report (credit report) from any consumer reporting agency and to obtain background information about me.

NOTE: Any broker gathering information about an applicant acts under specific instructions to verify some or all of the information described in this authorization. The broker maintains a privacy policy which is available upon request.
